I assume the heart honor lead offered in the poll is the normal attitude lead not the unblock lead.

I'll lead the top heart. Declarer should have a trick source here with clubs breaking onside. I'm not so worried about giving up a slow heart trick to declarer. In exchange I make when we can cash 5 hearts before they get in (including Jxx with partner and the stiff Q on the table) or when I can switch to diamonds when that's partner's real source (Kxxxx, xx, KQJ9, xx or so). Of course I'm paying off to their having 6 clubs to run and partner having 100 honors in spades and the DK (with the ace in dummy) or the DK where now they have time to clear hearts for the 9th trick. That implies they drove to game on 22 HCP though. This auction doesn't sound like it.

I think it's clear to lead either a heart honor or low card. I guess with no entry I'd lead low and hope partner has a minor suit stopper or hQ

I did lead a . 3NT-1 was also on the scoreboard. I guess with a low lead not ducked by East.
