OK, so east doesn't have the hand he represented in the bidding. This time, it worked out for them. Maybe next time, opening leader will hold the ♣AK and the result will be very different.
I am not going to lose any sleep over choosing a losing lead because my opponent bid badly. What goes around comes around.
What lead do you choose? Is it just a guess?

Nah, in a normal random auction east would rebid hearts.

All of 2♣, 2NT, 3♦, 3♥, 3NT and 4♥ are "normal" rebids in this context, no?

If you had agreed on that convention North had a Darkner double. "Partner, bid the usual(often unbid)suit." But we all know that such doubles only occur in Bridge World.

I agree with the comment, except that presumably you are referring to Cloud Cuckoo Land, and the meaning of that has become corrupted over the years. It was originally a perfect city in the clouds (in The Birds), and they would not have had this auction there. I am not sure, therefore, that one can do a simulation. And what should double of the final contract ask for? It doesn't seem that the traditional meaning of the first suit bid by dummy can be needed, so I would suggest that it shows something like AQ of spades and asks for them to be led. I therefore lead a heart.

6NT is not a bad contract. It will win on over 90% of the time that opening leader does not lead a club. Assuming a Bird-Anthias leader, the opponents will usually not lead the club unless leader has the AK of ♣, never from K led suit, only from the A led suit if J ♣ not held and 0 or 1 outside of the other J's are held, about 1 of 4 hands where neither A or K of clubs are held. Yes, 6♦ is the optimal contract at IMPs, and probably would have been at matchpoints. Note that 3 NT is a bad contract, the opponents have length in clubs, and it will in many auctions be the unbid or 4th suit. On actual hand 3NT would go down.
