What is double here?(You might ask what 5 ♥ means, but assume you know that east west does not have any firm agreement here..)

If you cant live with that explanation, assume that west bid 6♣ over 5♦.

either lead a spade or lead a diamond. I'm guessing diamond based on the penalty pass & lack of reopening X.

Opinions seem to vary about a Lightner double when the leader has shown a weak one-suiter.

One argument is that it asks for a lead of your long suit, because:
- That's the only suit where a ruff is possible.
- It's unlikely that they'll be in slam with two fast losers
- Without the double you might lead a side suit anyway.

The other is that:
- Leading your own suit is normal, so double asks for another suit (presumably a spade in this auction).
- If there is a ruff available in our suit, a double will just encourage them to bid 6NT, which will often make.
... that would still not be conclusive proof, before someone wants to explain that to me as well as if I was a 5 year-old. - gwnn

West clearly had a penalty pass, so he has ♦. South also has ♦. He wouldn't lead ♦ unless he's told to, and that's what North's Dbl means: lead something you normally wouldn't do = ♦.
