(I got it wrong first time but after some thinking I am not sure what's the best play).

A spade could be right if declarer holds for example xx AQJ10xxx Jxx x. This rather specific though and I am sure I would have tried the simple 2 return.

I'd give a club ruff, but ask for a spade return. That's necessary if declarer has xx AKQxxxx Jx xx or x AKQxxxx Jxx xx.
... that would still not be conclusive proof, before someone wants to explain that to me as well as if I was a 5 year-old. - gwnn

Why are you certain partner's ruffing this, why hasn't he led your suit from 43 ?

There are a couple of unlikely possibilities where a trump lead is required.

void, AQJxxxxx, Qxxx, x for example.

Not sure what the best practical chance is.

My first reaction is SK, setting up a 3rd trick my way, and hope partner has a trump winner.

The club-ruff play requires partner to have ANOTHER trump winner (Kxx or Qxxx in hearts), otherwise declarer will be taking pitches.

If partner returns after ruff there will be a trump promotion on a further club play so Kx or Qxx is enough.
