Opening 3S w/r seems terrible with or without the HQ

This is a 4S opening at nv vs vul and also at nv vs nv, Q of H or no Q of H.

You can hardly call this 0-7HCP, so I wouldn't open 3. Clear 4 opening at this vulnerability, again a clear pass as responder.

If you open 3 as solid as this, then I think a raise should be automatic.

Pass, but would surely have opened 4 nv vs vul. Even the rule of 1-2-3, which I tell my students is a tad too conservative, dictates a 4 opening.

yeah this is an = vulnerability 3 call. If you cannot resist preempting, 2 would be more likely to get you to game IMO.

oops I didn't consider 4 as a preempt; probably my choice of call at these colors.

Opener has: KQJT862 QT9 62 2

I basically work on the rule of 2-3-4 - so thats a 4 opener for me w/r. Failing that, opening it 1 makes more sense to me than 2 or 3 - partner will never read me for this much if I open 2 or 3.

I have seen this sort of hand opened 3 in 4th seat - though I am thinking probably one of the other players lost a picture card.
