Looks like the lead is from ♠KTx2.

In time I hope to make 3 spades, 1 heart, 3 diamonds and 2 clubs. I doubt I can make this if the ♥A is on my right, assuming they defend adequately.

So I'll just exit a middle spade. What happens now?
